There are a few things you can do before your consultation to maximize your consultation time and weight loss journey.

  1. Complete Medical History Intake Form here.
  2. Add your consultation to your calendar. Enable notifications so you’re sure to be on time and ready to get the most out of your time with your Medical Weight Loss Consultant from Dr. Tran’s team. 
  3. Make a list of questions and be ready with a notebook and pen so you can get answers to all of your questions during your consultation.
  4. Get a quick start on losing weight by emailing any lab or medical records Dr. Tran and her team will need to treat you to [email protected]. If you prefer, we can help you get your records after your consultation during your intake process.
